چکیده
*Occam’s razor, named after William of Ockham a Franciscan friar of the early 14th century, refers to the principle of logic that states that the simplest explanation is the most likely correct; in science with competing hypotheses, the one with the fewest assumptions is preferred. Hickam’s dictum is the counter-argument to Occam’s razor in medicine; the statement ‘Patients can have as many diseases as they damn well please’ attributed to John Hickam, a great US physician and educator, who originally published research on lung function in heart and lung disease.

A 61-year-old Chinese man with long-standing, stable Eosinophilic Granulomatosis with Polyangiitis (EGPA) and asthma, presented with acute hypoxemia and declining obstructive pulmonary function. Elevated serum IgE levels, positive Aspergillus fumigatus specific IgE and CT findings of central bronchiectasis with small airway mucoid impaction confirmed new development of Allergic Bronchopulmonary...

Previous reports of combined allergic bronchopulmonary mycosis (ABPM) and eosinophilic granulomatosis with polyangiitis (EGPA) are limited; however, among 41 cases of ABPM and 18 cases of EGPA treated at our hospital, we experienced three cases of combined ABPM and EGPA. In two of these cases, the diagnosis of EGPA preceded that of ABPM, however, one of the two cases had already shown findings ...

Aspergillus is a ubiquitous dimorphic fungus that causes a variety of human diseases ranging in severity from trivial to life-threatening, depending on the host response. An intact host defence is important to prevent disease, but individuals with pre-existing structural lung disease, atopy, occupational exposure or impaired immunity are susceptible. Three distinctive patterns of aspergillus-re...

Bronchocentric granulomatosis was first described in 19731 and is characterised by a necrotising granulomatous destruction of bronchial walls, bronchioles, and adjacent lung parenchyma. The bronchial lumen is filled by debris and cells, predominantly eosinophils in asthmatic patients and polymorphonuclear leucocytes in non-asthmatic patients.2 Proximal bronchi may show severe chondritis. Any va...
